What to expect
A credentialed pharmacist will visit the patient at home, to get an understanding of all the medicines in use, their current health and goals of care.
A medicine management plan will be developed to help you, the patient and their health practitioners manage medicines. This plan aims to simplify the regimen, decrease any adverse events and ensure that the medicines in use are aligned with the patient's health goals.
What it costs
A home medicine review is a free service to eligible patients that hold a Medicare or Veterans (DVA) card. It requires a referral from a General Practitioner (GP); Specialist in Pain Medicine; Specialist Physician; Specialist Psychiatrist; or Specialist in Palliative Medicine.
Common questions
- How do we access a HMR?
Speak to the patient's doctor to find out if a HMR would be potentially beneficial. If the patient meets the requirements of the referral criteria, then a referral can be written and a booking can be made.
- What is a Credentialed Pharmacist?
A credentialed pharmacist has completed extra study to specialise in quality use of medicines. They make evidence-based recommendations to the prescribing doctor, with the health goals of the patient in front of mind.
