Recent hospital discharge/changes to medicines
Suspected adverse effect from a medicine
Patient has multiple prescribers
Patient is not responding to therapy
Patient is having difficulty remembering to take medicine
Patient has concerns about their medicine
Polypharmacy (taking 5 or more medicines)

What should I include in my referral?

The clinical question that you would like answered
A list of medicine (including indications)
Social history and demographics, if relevant
Relevant issues e.g. cognitive impairment, vision or hearing deficits, reduced renal function
Medical and surgical history
Pathology results

What is a Credentialed Pharmacist?

A credentialed pharmacist has completed extra study to specialise in quality use of medicines. They make evidence-based recommendations to the prescribing doctor, with the health goals of the patient in front of mind.

When can I expect the report?

The interview is generally booked and completed within 14 days of the referral. If this is not possible, the credentialed pharmacist will notify the referring doctor of the expected interview date.
